Description of the patient's disability in order for us to recommend the best mode of transport
Oxygen Requirements
Location of the pick up - to include a room number if at a medical facility
Destination - facility name or address and room number if available
Facility or party responsible for payment
Next of kin or responsible party's name and telephone number if the transport is a self-pay
